CIRCULATION DISORDERS

in the legs, such as varicose veins (varices) or spider veins initially require a medical examination. In order to determine a malfunction of the veins, you need a special ultrasound examination (venous Doppler, duplex sonography). This examination can be carried out in our practice. Depending on the findings, we can advise and select the appropriate treatment methods (Laser Surgery, Laser Endoperivenous, Mini Phlebectomy, or Micro – Schaumverödung).

Of course, the practice has all the modern medical facilities. Numerous out day surgical treatments will be paid directly from the private insurance.

Treatment

The varicose vein examination is completely painless and is performed with modern Color duplex- ultrasound equipment and is the international standard. Radiological examination of the veins after injection of a radiopaque fluid (venography) is required only at selected venous diseases. In addition to color duplex examination, we offer further investigation in our practice as needed:

Doppler examination displays the blood flow in the vessels of the patient. It is used to assess the functional state of the veins and arteries.

Color Doppler ultrasound examination of the veins

The color duplex ultrasound is a combination of the conventional ultrasound imaging and the color-coded Doppler sonography (blood flow measurement). By means of color duplex sonography both superficial and deep vein are displayed.

Digital Photophlethysmography

This venous function study helps in the clarification of leg swelling or varicose veins. The patient sits in a chair, a sensor is adhered to both lower legs. The foot is raised rhythmically for 7-8 times. From the measured curves, the severity of the venous insufficiency can be determined. It also helps to differentiate between the superficial and deep venous system.

4 Channel Pulse Oscillography

Peripheral circulatory disorders are detected using 4-channel oscillography by simultaneous measurement at fingers and toes. Blood circulation variations before and after dilatations, peripheral vascular surgery, vascular training and medicaments’ therapy can be objectified.

Electronic Pulse Oscillography

The Electronics Pulse Oscillography by emission of a light beam measures the blood flow to the fingers or toes. For this, a sensor is clamped to the tip of the finger or toe. Small arterial disease and vascular spasms can be ascertained.

Diagnosis

Venous disorders are a widespread disease. Approximately 50% of persons older than 40 have more or less pronounced damage of the venous system, 25% suffer from varicose veins.
Symptoms: Heavy and tired legs – swelling – pain, night cramps – externally visible veins on the leg. If you have these symptoms, it is advisable to have a medical examination of veins. Depending on the findings, we can choose the appropriate treatment.

    • Laser treatment of varicose veins: One of the most advanced method of treatment of varicose veins instead of stripping operation.
    • Endo perivenous laser: For side branches and for spider veins and it causes photocoagulation of veins.
    • Mini Phlebectomy: The mini surgical removal of side branches is a gentle and cosmetically demanding procedure.
    • Veins foam sclerotherapy (Mikrosklerotherapie): This is a successfully proven treatment fon the disturbing spider veins.
    • Stripping operation: This procedure is performed under general anesthesia or spinal anesthesia and so it is possible only with hospitalization. Even if it is a classical surgical procedure, complications such as bleeding, lymphatic damage and nerve damage may occur. That’s why we prefer the laser treatment and is done as outpatient operation with local anesthesia.
Varicose Veins

The treatment of varicose veins using the most modern laser technology is a time and trouble saving method. This is an alternative to the traditional surgical treatment of varicose veins. It can be administered on outpatients in local anaesthesia. By laser surgery there is always an anesthesiologist present (standby).

Advantages of laser treatment:

  • No need for general anaesthesia
  • No need for inguinal incision
  • The surgical laser intervention is of very short duration; the pain is reduced to a minimum
  • Laser treatment brings excellent medical and cosmetic results and less recurrences
  • Duration of sick leave is very short
  • There is no need of hospitalisation at all
  • Laser operations can be done also in the summer months
  • No need to wear stockings more three weeks after laser treatment
Vein Laser Operation

Side varicose veins are the pathological enlargement of lateral branches of the trunk veins of the superficial venous system. They occur on the inside and outside of the upper and lower leg and often cause pain and cosmetic disorders. The treatments are Ultrasound guided Foam Sclerotherapy, Miniphlebectomy-Varady Method or Laser treatment.

Ultrasound guided foam sclerotherapy

The ultrasound-guided micro foam sclerotherapy is a minimally invasive method for side branches, recurrent varices and deep lying varices.

Miniphlebectomy-Varady Method

Miniphlebectomy-Varady Method is a mini surgical removal of side branches and is regarded as a gentle and cosmetically demanding procedure. The procedures can be performed under local anesthesia and on an outpatient basis. The varicose veins are removed through small incisions with Varady hooks. Healthy veins are left. This gives maximum protection of the lymph vessels and nerves surrounding the veins. The most important instruments in this operation is the Varady hooks. A resumption of daily activities is possible the next day.

Endoperivenous Laser Reticular Veins

Endoluminal perivenous laser is a valid and modern alternative to chemical sclerosis. This is a very effective method by deeper spider veins and reticular veins which cannot be usually treated trans dermally. To introduce the laser fiber, instead of local anesthesia, air cooling is done. Endolaser with micro fiber causes photocoagulation of the veins and is hardly absorbed by water and fat. It does not harm the surrounding peri venous tissue. The long-term result is the disappearance of the reticular veins.

Side Varicose Veins

Spider veins can be the first indication of a varicose disease. Therefore before treatment the entire superficial and the deep venous system should be examined. We offer the following treatments:

Micro foam sclerotherapy with Veinlite

Micro foam sclerotherapy with Veinlite is a proven treatment especially for the annoying spider veins. In the micro-sclerotherapy Aethoxysklerol in foam is injected with a very small needle into the vein. Mostly several treatment sessions to be performed.

KTP Laser for Spider Veins

Treatment with the laser (KTP Green Laser 532nm) is a method of treating aesthetically disturbing vascular dilations and small hemangioma. The laser is an alternative treatment option to other methods such as sclerotherapy and has a much lower scar risk with often better therapeutic success.

What is treatable?

Couperose: is a network of tiny veins on the nose or cheeks that turn red in the cold or heat. These can be well treated with the KTP laser.

Spider Veins: If the spider veins are very fine and reddish, they can be treated with a KTP vascular laser. Here usually 2-3 sessions are needed. You do not need patches and compression stockings afterwards.

Red microdots on the body: these points are usually made to disappear in a session with the KTP laser.

 

The procedure is generally less painful, so need for anesthetic cream. The diagnosis must always be clear before the laser therapy. Immediately before the procedure, no cosmetics or make-up should be applied.

After the procedure, redness, swelling, possibly crust formation, increased or decreased pigmentation may occur. The crusts should not be scraped off. The skin should be cooled for 5-10 min after a laser treatment.

Intensive sun exposure, solarium and self-tanner should be avoided approximately 2 weeks after treatment. The final cosmetic result can be assessed at the earliest after 6-8 weeks.

Most of the time, 1-2 treatments will be enough and the effect will last for several years. Should then again separate vessels occur, these can be treated easily again.

Spider Veins Treatment

Cutaneous veins on the back of the hand are superficial veins that run in the subcutaneous tissue and on the skin surface may be visible.
Due to the fat loss under the skin, these veins can be highly prominent and sometimes very annoying. In selected cases, it is possible to treat these veins using a laser.
